滋养细胞肿瘤患者子宫病灶大小与化疗疗程的关系

目的探讨滋养细胞肿瘤患者子宫病灶大小与化疗疗程的关系。方法应用彩色多普勒超声诊断仪探测了滋养细胞肿瘤２９例子宫病灶的大小、子宫血流情况，并与治愈时化疗疗程数作比较。结果随着子宫上病灶的增大，子宫动脉的搏动指数（ＰＩ）及阻力指数（ＲＩ）逐渐下降，病灶周围血流的搏动指数及阻力指数也逐渐下降，而治愈所需的疗程数逐渐增加，表明子宫病灶的大小、子宫血流情况与化疗效程之间有一定关联，而血清β－ｈＣＧ水平不能反映子宫病灶的大小及病情的严重程度。结论应用彩超测定子宫病灶及血流情况有助于估计滋养细胞肿瘤患者的预后，而β－ｈＣＧ水平与病情严重程度、预后不平行。

The preliminary study for the relationship with the uterine lesions and chemotherapy in trophoblastic tumors

To assess the reliability of the color Doppler flow imaging in the detection of uterine involvement, uterine circulation in cases of trophoblastic tumors, with comparison of patients' chemotherapeutic courses and to study the relationship among them.Methods The color Doppler flow imaging was performed in twenty-nine patients with the trophoblastic tumor at initial diagnosis. The uterine lesions and uterine artery waveform were detected. Sera β-hCG levels were measured with radioimmunoassay (RIA ) technique. Chemotherapeutic treatments were prescribed.Results Following the intramyometrial lesions being larger , the pulsatility index (PI)and resistence index (RI) of both the uterine artery and the perilesions' artery decreased,and the chemotherapeutic courses required to be increased. Sera β-hCG level could not indicate how much the uterine lesions were and what the clinical stages reached.Conclusion The application of the color Doppler flot flow imaging in cases with the trophoblastic tumors may be useful to assess patients' prognosis.
