灌注成像对脑内、外原发肿瘤的鉴别诊断价值

摘    要:目的 评价磁共振灌注加权成像 (PWI)对脑内、外肿瘤鉴别诊断的价值。方法  43例原发性脑内肿瘤、2 6例原发性脑外肿瘤病人行双倍剂量T2PWI,求出肿瘤和脑白质的相对脑血流容积 (rCBV)和平均通过时间 (MTT) ,计算肿瘤与脑白质的rCBV之比QrCBV、肿瘤的MTT较白质的延长值rMTT ,比较两组间QrCBV和rMTT有无差异。结果 原发性脑内、外肿瘤的QrCBV间无显著统计学差异 ,但原发性脑外肿瘤的rMTT较原发性脑内肿瘤延长 ,两者间差异显著 (P <0 .0 1)。结论 MRPWI在脑内、外原发肿瘤的鉴别诊断中具有重要价值。

Abstract:Purpose To evaluate the potential of perfusion weighted imaging (PWI) in differential diagnosis between primary extra and intra axial brain tumors. Methods Sixty nine patients with primary brain tumor (43 intra axial, 26 extra axial) underwent contrast material enhanced perfusion weighted MR imaging, relative cerebral blood volume (rCBV) and mean transit time (MTT) of tumors and white matter (WM) were calculated. Q rCBV that represented the rCBV ratio of tumor to WM and rMTT that was the difference MTT between the tumor and WM were obtained. The student t test was used to determine if there was a statistically significant difference in Q rCBV and rMTT between two groups. Results There was no statistically signifcant difference in Q rCBV between two groups, but the rMTT of primary extra axial tumors was longer than primary intra axial tumors with great significance. Conclusion PWI is very useful in differentiating primary extra axial tumors from intra axial tumors. [
