宫腔镜技术对早期子宫内膜癌的诊断意义

目的探讨宫腔镜诊断技术对早期子宫内膜癌的诊断意义。方法将120例子宫内膜癌患者分为甲、乙两组,甲组60例为研究组,乙组60例为对照组,比较宫腔镜下定点活检与传统分段诊刮术对早期子宫内膜癌宫颈管受累诊断符合率。结果宫腔镜下定点活检判断宫颈管受累符合率甲组为97.7%,乙组为91.6%。甲、乙两组术中腹腔冲洗液阳性检测率分别为8.3%和6.6%,两组比较无显著性差异。腹腔冲洗液阳性患者预后与合并高危因素有关。甲组5例腹腔冲洗液检测阳性者中无1例复发,乙组4例腹腔冲洗液检测阳性者均合并高危因素,2例复发。结论宫腔镜探查术未加剧腹腔细胞播散风险,其术前诊断率高于传统分段诊刮术,患者病情复发与宫腔镜探查术无直接关系。

Diagnostic Significance of Hysterscope Exploration in Early Endometrial Carcinoma

Objective To evaluate the diagnostic significance of hysterscope exploration in early endometrial carcinoma.Methods 120 cases of early endometrial carcinoma patients were equally divided into A and B group,group A as the study group and group B as the control group,to compare the diagnose accordance rate of cervical canal involvement between the sentinel biopsy with hysteroscopy and the traditional Dilation Curettage.Results The coincidence rate which determines cervical canal involvement by the hysteroscopic sentinel biopsy was 97.7% and 91.6%,for the group B.The postive rate of intraoperative peritoneal lavage fluid for A and B group was 8.3% and 6.6%,respectively.There were no significant differences between the two groups.The prognosis of those patients with postive peritoneal lavage fluid was related with high risk factors owned by them.There was no case recurrence in 5 patients with postive peritoneal lavage fluid in group A.The high risk factors existed in all 4 patients with postive peritoneal lavage fluid in group B,two of them relapsed.Conclusion The hysterscope exploration can not aggravate the spread of peritoneal cells,and the preoperative diagnosis with hysterscope exploration is higher than that with traditional DilationCurettage.There is no relationship between recurrence and hysterscope exploration.
