摘    要:目的:探索保护胸肌的改良后外侧小切口开胸技术在胸部肿瘤手术中的临床应用价值.方法:应用改良后外侧小切口开胸技术有选择性地对62例胸部肿瘤患者施行胸科手术.其中行肺楔形切除术7例,肺叶切除术29例,一侧全肺切除术6例,胸腺瘤摘除术3例、纵隔肿瘤摘除术9例、食管平滑肌瘤摘除术2例、食管下段癌根治性切除手术6例.结果:全组均顺利地完成了手术治疗, 无并发症发生,手术损伤小,开胸出血量少,疼痛不明显,术后住院时间短,肺功能及肩关节活动影响小,术后康复加快,明显减轻了患者的心理负担.结论:在严格掌握适应证的前提下,有选择地采用保护胸肌的改良后外侧小切口可使患者受益.

Abstract:Objective:To search for the clinical feasibility and advantages of muscle sparing modified posterolateral small incision thoracotomy in thoracic tumors surgery.Methods:Sixty-two tumor patients underwent invasive thoracic surgery by using modified posterolateral small incision thoracotomy.Results:The operations were successfully performed with good exposure.There was no intraoperative and post-operative complications,and the post-operative convalescence was much shortened.Conclusion:Modified posterolateral thoracotomy through the intercostal space reduces bleeding and trauma to muscles and ribs,so that the post-operative pain is much reduced,hence the post-operative complication reduced and the convalescence is smooth,thus this incision is less traumatic,simple,and cosmetic alternative compared with posterolateral thoracotomy.
