急性脑卒中与脑心综合征分析

目的探讨脑心综合征在急性脑卒中的发病率、临床特点及预后。方法对370例急性脑卒中患者入院后的心电图，心肌酶谱等进行动态观察，并对临床资料进行分析。结果脑心综合征在急性脑卒中的发生率为52．70％，出血性脑卒中的脑心综合征发生率明显高于缺血性脑卒中（P〈0．01），位于脑干、脑室、蛛网膜下腔的脑卒中明显高于位于脑叶、基底节区的卒中（P〈0．01）。脑心综合症的心电图主要表现为心律失常、心肌缺血等，心肌酶谱异常率49．74％。合并脑心综合症死亡率（29．74％）明显高于未合并脑心综合征的卒中患者（11．43％）。结论脑心综合征的发生与脑卒中类型及部位有关，其预后较未合并脑心综合征的卒中患者差。脑卒中患者在治疗原发病的基础上积极防治脑心综合征。

Analysis of Cerebrocardiac Syndrome in Acute Apoplexy

Objective To discuss the morbidity,clinical features and prognosis of cerebrocardiac syndrome in acute apoplexy.Methods Dynamic observation was made to the ECG and myocardial zymogram of 370 hospitalized cases with acute apoplexy and an analysis was made to their clinical data.Results The occurrence of cerebrocardiac syndrome in acute apoplexy was 52.70%,the occurrence of cerebrocardiac syndrome in hemorrhagic apoplexy was much higher than that in ischemic apoplexy(P<0.01),the occurrence of apoplexy located in brain stem,ventricle and subarachnoid cavity was much higher than that located in lobe of brain and basal ganglia(P<0.01);the main ECG manifestations of cerebrocardiac syndrome were arrhythmia and myocardial ischemia,the myocardial zymogram of 49.74% cases were abnormal,the death rate in the cases complicated with cerebrocardiac syndrome(29.74%) was much higher than that in the cases without the complication of cerebrocardiac syndrome(11.43%).Conclusion The occurrence of cerebrocardiac syndrome is related with the type of apoplexy and its location,the prognosis of the patients complicated with cerebrocardiac syndrome is poorer than that of the patients without the complication of cerebrocardiac syndrome;it is important to prevent the occurrence of cerebrocardiac syndrome while treating the protopathy of the cases with apoplexy.
