多层螺旋CT后处理技术在早期肺癌诊断中的应用

摘    要:目的 探讨多层螺旋CT Myrian影像后处理技术在早期肺癌诊断中的应用效果。方法 回顾性选取温州医科大学附属金华医院2020年1~12月收治的78例怀疑为早期肺癌患者为研究对象,均应用Myrian影像后处理技术进行早期肺癌定性诊断和危险度评估,并与病理学诊断结果进行比较。结果 78例疑似患者经病理学确诊为早期肺癌48例,良性结节30例;48例早期肺癌患者淋巴转移9例。Myrian影像后处理技术三维重建肺结节显像对短毛刺征、棘突征、分叶征、血管集束征、胸膜凹陷征的显像效果优于CT平扫(P<0.05)。Myrian影像后处理技术诊断早期肺癌的敏感度、特异性、阳性预测值、阴性预测值均显著高于CT平扫(P<0.05);Myrian影像后处理技术诊断淋巴结转移的敏感度、特异性均显著高于CT平扫(P<0.05)。结论 与CT平扫相比,多层螺旋CT Myrian影像后处理技术在早期肺癌诊断中具有更好的诊断效能,为肺结节临床决策提供科学依据。

Application of multi-slice spiral CT post-processing technology in the diagnosis of early lung cancer
Abstract:Objective To explore the application effect of multi-slice spiral CT Myrian image post-processing diagnosis technology in the diagnosis of early lung cancer. Methods From January to December 2020, 78 patients with suspected early-stage lung cancer were selected from Jinhua Hospital Affiliated to Wenzhou Medical University and retrospectively analyzed. All of them applied Myrian image post-processing diagnosis technology to analyze the risk of early lung cancer and compared the results with the pathological examination results. Results Seventy-eight patients with suspected early lung cancer were pathologically diagnosed as 48 early lung cancers, 30 benign nodules, and 9 of the 48 early lung cancers had lymphatic metastasis. Myrian image post-processing technique showed better imaging effect on pulmonary nodules in patients with short burr sign, spinous process sign, lobular sign, vascular cluster sign and pleural indentation sign than plain CT scan (P<0.05). The sensitivity, specificity, positive predictive value and negative predictive value of Myrian image post-processing in the diagnosis of early lung cancer were significantly higher than those of plain CT (P<0.05). The sensitivity and specificity of Myrian image post-processing in the diagnosis of lymph node metastasis were significantly higher than those of plain CT (P<0.05). Conclusion Multislice spiral CT Myrian image post-processing has better diagnostic efficacy in early lung cancer diagnosis than plain CT, providing scientific basis for clinical decision making of pulmonary nodules.
