新生儿缺氧缺血性脑病的疗效观察(186例回顾性分析)

目的总结分析我科3年来新生儿缺氧缺血性脑病(HIE)的系统治疗方法及相应近期疗效和远期预后,探讨改善预后的措施。方法对186例HIE患儿在各个阶段进行全面系统治疗,对临床资料和随访结果进行回顾性分析。结果186例HIE中足月儿149例(80.1％),早产儿37例(19.9％)。病死率7.5％(重度HIE20.5％,中度5.1％,轻度0)。随访109例,后遗症发生率16.5％(重度HIE60.7％,中度6.1％,轻度0)。早产儿HIE者中度预后不良占37.0％,重度60.7％。结论重度和早产儿HIE预后较差。针对不同阶段病情特点给予综合治疗,可显著改善患儿预后。

Observation of curative effect of neonatal hypoxic-ischemic encephalopathy:retrospective analysis of 186 cases

Objective To summarize and analyze the systemic treatment, short-term curative effect, long-term prognosis of the neonates with hypoxic-ischemic encephalopathy(HIE) and explore the therapy to improve the prognosis. Methods Clinical data and follow-up outcomes of 186 neonates with HIE, which underwent comprehensive and systemic treatments, were analyzed retrospectively. Results There were 149(80.1 %) full-term and 37(19.9 %) premature infants. The mortality rate was 7.5 % (20.5 % in severe HIE, 5.1 % in moderate HIE). Among 109 survivors with follow-up, the sequelae rate is 16.5 % (60.7 % in severe HIE, 6.1 % in moderate HIE, and 0 in slight HIE.). Premature infants with HIE had a poor prognosis in 37.0 % of moderate HIE and 60.7 % of severe HIE. Conclusion Prognosis of both neonates with severe HIE and premature infants are bad. According to the characteristic at the different stages of the illness, comprehensive treatment could improve the prognosis for neonates with HIE.
