Ⅰ期非小细胞肺癌患者淋巴结角蛋白的检测及其意义

摘    要:目的探讨Ⅰ期非小细胞肺癌(non-small cell lung cancer,NSCLC)淋巴结中角蛋白的检测及其临床意义。方法应用快捷免疫组织化学方法检测40例Ⅰ期NSCLC患者和10例肺良性病变患者淋巴结中角蛋白表达情况。结果40例NSCLC患者中,有11例患者28枚淋巴结角蛋白检测阳性,阳性率为14.6%;而10例肺良性病变患者淋巴结角蛋白检测均阴性,两者比较,差异有统计学意义(P〈0.05)。在Ⅰ期NSCLC患者的192枚淋巴结中,应用快捷免疫组织化学方法检测28枚淋巴结有淋巴结微转移,阳性率为14.6%;而常规病理检查无淋巴结微转移,两者比较。差异有统计学意义(P〈0.05)。结论应用快捷免疫组织化学方法检测Ⅰ期NSCLC患者淋巴结中角蛋白的表达,有助于诊断淋巴结微转移,对预测患者术后复发、判断预后、指导治疗具有重要意义。

Abstract:Objective To detect the cytokeratin status in lymph nodes from patients with stage Ⅰ NSCLC and explore its clinical significance. Methods The cytokeratin expression was detected in lymph nodes from 40 patients with Ⅰ NSCLC and 10 with benign pulmonary lesion by immunohistochemical staining. Results Positive cytokeratin was detected in lymph nodes in 11 of 40 patients with Ⅰ NSCLC,the positive rate of Lymph node was 14.6%. while the detected result of cytokeratin in lymph nodes in 10 patients with benign pulmonary lesion was negative(P〈0.05). Micrometastasis were detected negative in 192 lymph nodes from I NSCLC by traditional pathological method, while 28 cases were detected positive by immunohistichemical staining and the positive rate was 14.6 % (P〈0.05). Conclusion Micrometastasis in lymph nodes of patients with I NSCLC could be diagnosed by investigating the expressions of cytokeratin. Detection of cytokeratin might be helpful to predict cancer recurrences after surgical resection , and provide important clinical information for the evaluation of prognosis and in the selection of appropriate treatment strategies.
