低场MRI与CDFI在膝关节软骨退行性变分期评价中的对比研究

摘    要:目的:探讨低场核磁共振成像(magnatic resonance imaging,MRI)与彩色多普勒超声(color flow doppler imaging,CDFI)诊断膝关节软骨退行性变分期与关节镜检查或手术分期的相关性?方法:对58例膝关节镜检查或手术的患者,术前采用低场MRI观察股骨内?外髁?股骨滑车正中沟?滑车内侧斜面和滑车外侧斜面,共计290个关节面?其中32个膝关节同时进行CDFI检查?将低场MRI和CDFI判断膝关节软骨退变的分期与关节镜分期结果对比分析?结果:以关节镜诊断为标准,低场MRI与CDFI评判膝关节软骨退变的分期与关节镜分期的相关系数分别为0.800 (P < O.05)和0.667(P < O.05)?二者联合检查,与关节镜分期的相关系数为0.821(P < 0.05)?结论:低场MRI与CDFI诊断膝关节软骨退变的分期与关节镜分期均有较好的相关性,有助于评估病情,低场MRI与CDFI联合应用,可更好地评价病变的严重程度,指导临床治疗,值得在基层医院推广?

Abstract:Objective: To evaluate the correlation between low field strength MRI and/or color flow doppler imaging(CDFI) and the arthroscopic or intra-operative staging of cartilaginous degeneration of knee joints. Methods: Fifity eight patients for knee joint arthroscopic examination or operation were included. A total of 290 articular surfaces were examined by low filed MRI to evaluate the lateral and medial epicondyle of femur,the intercondylar fassa,and the lateral and medial slope of trochlea. Among them,32 joints were received CDFI examination. Results of MRI and CDFI examination were comparied with that of arthroscopic examination. Results: Taken the arthroscopic examination as gold standard,the correlation index of MRI and CDFI was 0.800(P < O.05) and 0.667(P < O.05),respectively,while combined MRI with CDFI,the correlation index was 0.821(P < 0.05). Conclusion: Both low field MRI and CDFI have good correlation with arthroscope in the diagnose and staging of cartilaginous degeneration of knee joints. The correlation index of combination is higher than single modality. These results implied a role of low field MRI and CDFI in the diagnosis and preoperative assessment of cartilaginous degeneration of knee joints
