L-精氨酸对缺血再灌注骨骼肌的影响

摘    要:目的 探讨L-精氨酸对缺血再灌注骨骼肌的影响。方法 选用健康新西兰大白兔20只,建立左后肢缺血模型。缺血3.5 h的肢体恢复血流前分别在30 min内自腹壁浅动脉灌注林格氏液、含L-精氨酸的林格氏液各15 ml,然后恢复肢体血液循环3 h。根据灌注液的不同将20只大白兔分为对照组、实验组。实验结束后测量左右胫前肌最大强直收缩张力、抽取左侧股静脉血测定血清CK、LDH、NO、cGMP,取左胫前肌标本作骨骼肌干湿重比测定。另取健康新西兰大白兔10只抽取左侧股静脉血测定血清NO、cGMP。结果 实验组血清NO为117.169±11.569、cGMP为0.317±0.030均接近正常值,左右胫前肌等长张力之比为59.252±8.612、血清CK为35027.74±3352.13、LDH为804.41±413.82较对照组有显著性改善。结论 L-精氨酸能够显著地提高血清NO、cGMP含量并有效地保护缺血再灌注骨骼肌。

Abstract:Objective To explore the protective effects of L-arginin on skeletal muscle with ischemia-reperfusion. Methods Twenty healthy New Zealand rabbits were chosen and utilized to set up ischemia models in their left hindlimbs. With the hindlimbs kept ischemic for 3.5 hours, 15 ml of different solutions, i. e. Ringer's solution., Ringer's solution containing L-arginin were respectively perfused through superficial epigastrica artery into the ischemic limbs in a period of 30 minutes. And then the blood circulation was restored in the limbs and maintained for 3 hours. Accordingly the 30 rabbits were devided into 2 groups (control group, L-arginin group). Isometric twitch contractive forces of both the right and left anterior tibialis muscles were measured 3 hours after reperfusion. Muscles samples were taken from left anterior tibialis to measure the muscle dry to wet weight ratios. Blood samples were collected from left femoral veins to measure the serum CK LDI-K NCX cGMP in serum. Results In L-arginin group, the levels of NCK cGMP in serum were 117.169 +11.569, 0.317+0.030, close to normal, muscle isometric twitch contractive force ratios between left and right anterior tibialis was 59.252 +8.612, CPK, LDH in serum were 35027.74 + 3352.13?804.41 + 413.82 differently, significantly improved than in the control group. Conclusion L-arginin can significantly arise the levels of NCK cGMP in serum and effectively protect skeletal muscle from ischemia-reperfusion injury.
