糖尿病性视网膜病变手术干预及围手术期止血药物应用

目的观察增殖性糖尿病性视网膜病变（PDR）患者行玻璃体手术前及术后分别使用止血药物对术中、术后视网膜玻璃体出血情况的影响,探讨PDR围手术期止血药物使用的最佳方案。方法分析我院2006年1月~2008年12月收治的有完整随访观察资料的PDR患者43例（49眼）按止血药物使用方案不同分为3组,A组14例（17眼）,术前1h肌注注射用血凝酶1KU,B组13例（15眼）,术后静脉滴注氨甲环酸0.75mg,1次/d,5d,C组16例（17眼）既术前肌注注射用血凝酶1KU,又于术后静脉滴注氨甲环酸0.75mg,1次/d,5d。对比分析3组术中术后出血情况。结果术中渗血程度A、C组差异无统计学意义（P〉0.05）,均优于B组,术后出血指标（术后早期玻璃体混浊程度、晚期再出血）,C组优于A、B组（P〈0.05）,A、B组差异无统计学意义（P〉0.05）。结论围手术期即术前术后均使用止血药有利于减少术中出血及术后再出血,效果优于单纯术前或术后使用止血药。

Study of hemostyptic schedule for proliferative diabetic retinopathy before and postvitrectomy
