摘    要:目的探讨术中实时超声联合神经导航在显微神经外科切除颅内巨大脑膜瘤中的作用和价值。方法通过对33例术中实时超声联合神经导航切除颅内巨大脑膜瘤患者及32例对照组患者的回顾性分析,评价术中实时超声在确定颅内巨大脑膜瘤的定位、边界及手术切除范围,并监测术后肿瘤是否有残余,随访患者术后情况。结果术中实时超声对颅内巨大脑膜瘤的发现率达100%,超声组患者全切率90.9%(30/33),对照组全切率75.0%(24/32);超声组患者术后1周KPS评分及住院时长与对照组比均有显著统计学差异(P〈0.05)。结论神经外科术中实时超声检查可安全有效的定位颅内脑膜瘤边界,监测肿瘤残余情况并最大限度减小正常脑组织损伤,保护神经功能,有利于患者术后恢复,具有重要的临床应用价值。

Application value of intraoperative ultrasonography in resection of giant intracranial meningio- mas

Abstract:Objective To evaluate application value of intraoperative ultrasonography in the resection of giant intracranial meningioma. Methods 65 patients with giant intracranial men- ingioma were divided into ultrasonography group (33) and control group (32). Intraoperative ul- trasonography was performed to determine tumor location,range of excision and remnant tumors after resection. Results In ultrasonography group,display rate of lesions wasl00%. The total re- section rate was significantly different between ultrasonography group (90. 9%, 30/33) and con- trol group (75.0~, 24/32,P~0. 05). It is significant different between two groups in KPS scores and length of stay (P%0.05). Conclusion Intraoperative ultrasonography helps surgeons identify the location and boundary of intracranial meningioma and preserve the cerebral functional areas. It enhances the security and efficacy of there section of cerebral meningioma and becomes one of im- portant assistant approaches in neurosurgery.
