四肢主干动脉损伤的诊治（附61例临床分析）

目的：探讨分析四肢主干动脉损伤的诊治、方法。对61例四肢主干动员伤进行回顾性分析，其中急性动脉伤56例，假性动脉瘤5例，根据伤情分别采用单纯修补受损血清。对端吻合，自体静脉移植或人造血管移植修复血管损伤，结果：截肢2例，其中1例截肢后死于肾功能衰竭，其科59例肢体存活并恢复功能，结论：（1）早期诊断，正确处理，尽快恢复血清环是挽救肢体的关键；（2）动脉缺损2cm以上需行血管移植，尽量采用自体静脉移植，必要时可使用人造血管。

Diagnosis and treatment of major arterial injuries in limbs: clinical analysis of 61 cases

Objective: To investigate the diagnosis and treatment of major arterial injuries in limbs. Methods: Sixty-one cases with major arterial injuries in limbs were reviewed, of those, 56 cases were acute injury and 5 cases were pseudoaneurysms.The managements were direct suture, end to end anastomosis, and reconstruction by autogenous vein or prosthetic material. Results: Amputation had been done in 2 cases,and one of them was died of renal failure. The other 59 cases all had their limbs surrived and got good function.Conclusion:(1)Early accurate diagnosis and prompt surgical treatment of the injured arteries to restore circulation of the limb are the curcial factors for a successful limb salvage.(2) Graft implantation must be performed when the vascular injury was longer than 2cm. Autogenous vein was the first choice of graft material. Prosthetic material was another choice when necessary.
