对主诉青霉素阳性患者的临床再探讨

目的　对 12 3例以往青霉素阳性患者 89例青霉素皮试阳性者再分析。方法　对 89例青霉素皮试阳性者运用 0 .2 %利多卡因作为溶液配制剂进行测试 ,定人观察 ,同时避免了在皮试液的温度、浓度、皮试部位、病人心理等方面易发生假阳性的因素。结果　在 89例青霉素皮试局部阳性者中 ,青霉素阴性为 69例 ,注射青霉素后有阳性反应者 14例 ,有阳性症状者 6例 ,分别占77.5 1%、15 .7%、6.85。结论　假阳性率的产生与测试者、皮试液、病人三者均有一定的联系 ,皮试时要规范操作、爱岗敬业 ,可减少假阳性的产生

Clinic Investigation in Patients with Complaints of Positive History of Penicillin

Objectives To analyse the re-tested results of 89 patients with positive history of penicillin skin-test. Methods 89 patients with complaints of positive history of penicillin skin-test were involved and re-tested in our investigation. 0.2% lidocaine was used as solvent in our skin-test. To avoid the difference coming from temperature and concentration of drug, injection site, patient's psychology or other factors, the same nurse performed the injection and observation. Results The results of penicillin skin-test in 89 patients showed that: 69 cases(77.5%) were negative , only 6 cases(6.8%) appeared positive symptoms and 14 cases(15.7%) were positive. Conclusions The false positive result is related with the operator, the drug, and even the subjective sense of patients. More efforts can reduce the rate of false positive.
