改良塞丁格尔技术PICC置管在乳腺癌患者术后化疗中的应用

目的探讨改良赛丁格尔技术PICC置管在乳腺癌术后化疗的应用方法和价值。为化疗患者建立无痛、安全、有效的静脉通道,避免反复静脉穿刺,防止频繁采血及化疗药物外渗引起的静脉炎及局部组织坏死。方法对我院2009年9月至2011年12月255例术后需长期静脉输液化疗的乳腺癌患者行健侧PICC插管,并观察其临床效果。其中204例经肘窝外周静脉穿刺,51例行改良赛丁格尔技术穿刺。结果传统PICC置管的一次置管成功率为95%,导管留置时间为26～112d,平均为69d,少数患者出现轻微并发症,经积极处理后均好转。赛丁格尔技术PICC置营的一次置管成功率为99%,导管留置时间为34～146d,平均为83d,无并发症发生,两组患者比较,差异有统计学意义(P<0.05)。结论 PICC置管操作快速、方便,穿刺成功率高,维护简单,导管留置时间长,并发症低,为乳腺癌患者术后化疗用药提供了一条安全、方便的输液途径,值得临床推广应用。

Improved Sedinggeer technology PICC catheter in the postoperative chemotheraphy in breast cancer patients

Objective To investigate the improvement Saidinggeer PICC catheter application and value in breast cancer after chemotheraphy. Painless, safe, effective intravenous access for the patients treated with chemotherapy, to avoid repeated venipuncture, to prevent the frequent blood sampling and chemotherapy drug extravasation due to phlebitis and tissue necrosis.Methods Contralateral PICC intubation of the hospital in 2009 in September to December 2011 255 cases of postoperative requires long-term intravenous infusion chemotherapy for breast cancer patients, and to observe their clinical effect. 204 cases by the cubital fossa in the peripheral venous puncture, 51 patients improved Saidinggeer technology puncture.Results The success rate of 95 percent traditional PICC catheter once catheter, catheter indwelling time of 26 to 112 days, an average of 69 days, few patients had minor complications, after actively improved. PICC set camp in Saidinggeer technology was a catheter success rate of 99%, catheter time of 34 to 146 days, an average of 83 days, no complications occurred, two groups of patients, a significant difference （P〈0.05）.Conclusion PICC catheter is rapid, convenient, puncture a high success rate, easy maintenance, long catheter, low complication, postoperative chemotherapy drugs for breast cancer patients to provide a safe, convenient way of infusion, worthy of clinical application.
