复方丹参在肠缺血-再灌注损伤中保护作用的实验研究

摘    要:目的 :探讨肠缺血再灌注 (ischemia- reperfusion,I/ R)过程中复方丹参对肠缺血再灌注损伤的保护作用。方法 :采用肠系膜上动脉 (SMA)夹闭 1h后松夹造成缺血再灌注损伤作为动物模型。将 4 2只 SD大鼠随机分为假手术组、缺血再灌注组和复方丹参治疗组。假手术组分离 SMA但不夹闭。余各组分别于缺血即刻、缺血 1h和再灌注1h3个时点应用免疫组织化学的方法观察小肠 ICAM- 1表达 ,同时检测组织髓过氧化物酶活性和组织水含量等指标 ,并行苏木素 -伊红 (HE)染色于光镜下观察小肠组织病理形态学改变。结果 :免疫组织化学显示 ICAM- 1蛋白表达在缺血再灌注组肠缺血 1h及再灌注 1h组较假手术组、复方丹参治疗组增多 (P<0 .0 5 ) ,组织髓过氧化物酶活性同时升高 (P<0 .0 5 )。HE染色显示 ,I/ R损伤后 ,复方丹参治疗组肠屏障功能损伤较缺血再灌注组减轻。结论 :肠缺血再灌注时肠血管内皮细胞 ICAM- 1表达增加 ,由此介导的中性粒细胞在局部聚集、活化可能是肠粘膜上皮损伤和肠通透性增加的病理生理学基础。复方丹参通过抑制 ICAM- 1的表达对肠缺血再灌注损伤有保护作用。

Abstract:objective:To investigate the effect of composite salvia miltiorrhiza injection on rat intestinal ischemia-reperfusion injury.Methods:Rat intestinal I/R injury produced by occlusion of the superior mesenteric artery (SMA) for 60 minutes followed by reperfusion used as animal model.Fourty-two SD rats were randomly divided into sham-operation group, ischemia-reperfusion group and composite salvia miltiorrhiza injection treated group. In sham-operation group,SMA were separate but without occlusion. Tissue samples were taken at 2h after ischemia in sham-operation group , at 1h,2h after reperfusion in the other groups respectively. The expression of ICAM-1 in gut was assayed with immunohistochemistry. Myeloperoxidase (MPO) activity and water content of intestine were determined as well.Rescult:The ICAM-1 protein highly express at 1h post- ischemia and 1h reperfusion; MPO activity was enhanced (P<0.05) in accordance with ICAM-1 in ischemia-reperfusion group as compared with those in sham-operation group and treated group (P<0.05).The improvement of histological structures was observed after reperfusion injury.Conclusions:The aggregation and activition of PMN in intestine tissue is associatesd with the high expression of ICAM-1 on intestinal vascular endothelial cells in gut ischemia-reperfusion rats,which might be the mechanisms underlying intestinal mucosa injury and enhanced intstinal permeability. Composite salvia miltiorrhiza injection has a protective effect on intestinal I/R injury by way of ICAM-1 expression down-regulation.
