经前路植骨同期内固定术治疗胸腰椎结核

目的探讨经前路病灶清除植骨同期内固定手术治疗胸腰椎结核的临床价值。方法对49例胸腰椎结核患者采用同期前路内固定14例,非内固定35例,对两组病例的疗效、并发症、病灶复发、后凸畸形矫正结果和随访的变化进行对比分析。结果两组病例症状均明显改善,固定组植骨全部融合,病灶无复发,后凸畸形矫正明显优于非固定组,且卧床时间短,后凸畸形无加重。结论经前路病灶清除植骨同期内固定手术是治疗胸腰椎结核的可靠方法。

Bone graft and internal fixation via front route for treatment of thoracolumber vertebra tuberculosis

Objective To discuss the clinic merit of focus clearance,bone graft and internal fixation via the front approach at the same time for treating thoracolumber vertebra tuberculosis.Methods 49 cases of thoracolumber vertebra tuberculosis were operated upon from Jan.2002 to Jun 2006.Among them 14 cases were treated by front route internal fixation and 35 cases were not.The efficacy,complications,focus recurrence,rectification to posterior protrudent deformity were compared between the two groups.Results Symptoms of both groups were obviously improved.The planted bones were all fused.There was no focus recurrence.Rectification to protrudent deformity was superior in fixation group to nonfixation group.Besides,patients undergoing internal fixation had shorter bed-bound time.Conclusion It is feasible and reliable to treat thoracolumber vertebra tuberculosis by focus clearance,bone graft and internal fixation.
