非创伤性玻璃体积血的病因分析及手术疗效

目的分析非创伤性玻璃体积血的常见病因及探讨玻璃体切割手术对玻璃体积血的疗效.方法对1998年7月至2000年9月于我科行玻璃体切割手术治疗有随访记录47例51眼非创伤性玻璃体积血其病因构成,解剖、功能成功率进行回顾性分析.结果糖尿病视网膜病变玻璃体积血18眼、视网膜静脉周围炎玻璃体积血16眼、视网膜静脉阻塞玻璃体积血12眼,共占90.20%,为非创伤性玻璃体积血的主要原因.术后平均解剖成功率为94.12%,功能成功率为60.78%,治疗前后视力比较有显著差异(P＜0.01).结论糖尿病视网膜病变、视网膜静脉周围炎和视网膜静脉阻塞是玻璃体积血的主要病因.玻璃体切割手术是治疗玻璃体积血的有效方法.

Clinical analysis of the etiological factors in non-traumatic vitreous hemorrhage eyes and the clinical outcomes of vitrectomy

Objective To analyze the etiological factors in non traumatic vitreous hemorrhage and to investigate the clinical outcomes of vitrectomy in non traumatic vitreous hemorrhage eyes. Methods The etiological factors, anatomic success rate, and recovery rate of visual functions in a total of 47 patients (51 eyes) undergoing vitrectomy with follow up records from July 1998 to September 2000 in our department were analyzed retrospectively. Results Vitreous hemorrhage was found in 18 eyes of diabetic retinopathy, 16 eyes of Eales' disease, and 12 eyes of retinal vein occlusion, amounting to 90.20% of the total eyes. The anatomic success rate and recovery rate of visual functions after operation were 94.12% and 54.90%, respectively. There was significant difference in visual acuity before and after operation ( P <0.01). Conclusion Diabetic retinopathy, Eales disease, and retinal vein occlusion are the main etiological factors in non wound vitreous hemorrhage. Vitrectomy is an effective way for vitreous hemorrhage.
