摘    要:目的了解我院常见革兰阳性菌耐药情况,为临床合理选用抗菌药物提供参考依据。方法收集我院2008-2010年临床中检出的革兰阳性菌,分析其分布及耐药情况。结果 3年中共检出革兰阳性菌3 137株,前3位分别是凝固酶阴性的葡萄球菌(2 039株,65.0%)、金黄色葡萄球菌(662株,21.10%)、粪肠球菌(211株,6.73%),其中耐甲氧西林的葡萄球菌共2 154株,占68.66%。未发现对万古霉素、利奈唑胺、替考拉宁耐药的菌株,但对常用抗菌药物耐药性较高。结论革兰阳性菌耐药情况严重,应严格掌握抗菌药物使用指征,采取有效措施控制其使用强度,提高细菌监测水平,对减少耐药菌的发生及合理选用抗菌药物有着重要意义。

Abstract:Objective To investigate the resistance of common gram-positive bacteria, and propose rational use of antibiotics in clinic practice. Methods The gram-positive bacteria was collected from the patients culture in our hospital from 2008 to 2010, and the drug resistance was analyzed. Results 3 137 strains were detected, there were 2 039 strains of coagulase-negative staphylococci (65.0%) ,662 strains of staphylococcus aureus (21. 10% ) ,211 strains of enterococcus faecalis ( 6. 73 % ), of which, 2 154 ( 68.66 % ) methicillin-resistant staphylococcus aureus were detected. No resistant strain to vancomycin, linezolid and teicoplanin was found, however, the resistance to commonly used antibiotics was higher. Conclusion The resistance of gram-positive bacteria is serious, antimicrobial drug use should be strictly controlled, effective measures to control the intensity of antimicrobial drug should be taken, as a result, the level of bacterial monitoring could be improved, and it has significance on reducing the incidence of drug resistant and rational use of antibiotics.
