高血压早期肾损害患者血清Klotho蛋白及基因多态性的相关性

摘    要:目的探讨原发性高血压并早期肾功能损害患者血清Klotho蛋白水平及与Klotho基因F352V多态性的相关性。方法原发性高血压患者根据是否合并早期肾损害分为高血压组(A组)68例,高血压并早期肾损组(B组)52例,健康对照组70例。酶联免疫吸附法测定血清Klotho蛋白水平,应用等位基因特异性引物PCR技术检测Klotho基因F352V多态性。结果 Klotho基因F352V多态性FF、FV和VV在3组的分布频率有显著差异(χ2=21.633,P〈0.001)。高血压组V等位基因频率高于对照组,差异有统计学意义(χ2=6.389,P=0.011)。高血压并早期肾损组各基因型分布频率与对照组比较差异有统计学意义(χ2=18.666,P〈0.001),V等位基因频率高于对照组,差异有统计学意义(χ2=20.763,P〈0.001)。高血压组和高血压并早期肾损组等位基因分布差异也有统计学意义(χ2=4.849,P=0.028)。3组各基因型的血清Klotho蛋白浓度差异均有统计学意义(F值分别为21.754、27.295和22.139,均P〈0.01),FF、FV和VV三种基因型血清Klotho蛋白浓度两两比较差异均具有统计学意义(P〈0.05)。结论血清Klotho蛋白浓度增加可能是高血压及早期肾损害的保护性因素。Klotho基因多态性可能与高血压及早期肾损害的发生有关,VV基因型易患高血压,FF基因型可能对高血压及早期肾损害有保护作用。

Abstract:Objective To investigate the relationship between single nucleotide polymorphisms of Klotho gene and serum Klotho protein levels in EH patients with early renal damage.Methods Allele-specific primer(ASP) PCR technique was used to detect genotype of the Klotho gene three sites of SNPs.Sixty-eight essential hypertension(EH)patients,52 EH with early renal damage(EH-ERD)patients and 70 controls were enrolled into this study.ELISA was used to detect the serum level of Klotho protein.Results The distributions of Klotho gene polymorphism in the three groups showed a significant difference(χ2=21.633,P0.001).The difference was found between the frequency of genotypes in EH patients and EH patients with early renal damage(χ2=18.666,P0.001).In the three groups,the differences of frequencies of V allele were significant(P0.05).Conclusion The increase of Klotho protein concentration might be a protective factor of hypertension and early renal damage caused by hypertension.Klotho gene F352V polymorphism is associated with EH and EH-ERD.VV gene type is liable to hypertension,FF genetype is probably protective for early renal damage.
