改良背阔肌皮瓣游离移植的临床应用

目的探讨改良背阔肌皮瓣在体表巨大组织缺损修复中的应用。方法应用改良背阔肌皮瓣游离移植修复体表巨大组织缺损7例,其中头顶部2例,下肢5例。皮瓣切取过程中保留胸背神经和部分背阔肌,形成改良背阔肌皮瓣。结果皮瓣全部存活,受区创面修复基本满意,供区背阔肌功能得以保留。结论改良背阔肌皮瓣可覆盖面积大,血供可靠,且最大程度保留了供区背阔肌的功能,在体表巨大组织缺损的修复中具有较高的应用价值。

Clinical application of improved latissimus dorsiperforator flap

Objective To evaluate the clinical application of improved latissimus dorsi flap in large tissue defect repair.Methods Improved latissimus dorsi flaps were used in the 7 cases with large tissue defect, including 2 cases of parietal region repair, and 5 cases of lower limb wound repair.Thoracodorsal nerve and partial latissimus dorsi were retain during the flap being procedure, and improved latissimus dorsi flap was formed.Results All improved flaps were survived with satisfactory results （tissue defect repair）.the function of latissimus dorsi was well preserved postoperatively. Conclusion Improved latissimus dorsi flap is characterized by the large area of tissue and reliable blood supply ,and especially the function of latissimus dorsiperforator were preserved in essence. which has a good value for application to repair large tissue defect.
