维甲酸受体β在膀胱移行细胞癌中的表达及意义

目的检测维甲酸受体β（RARβ）在膀胱移行细胞癌中的表达，并探讨其意义。方法应用免疫组织化学方法对80例膀胱移行细胞癌组织标本中RARβ表达进行检测：男56例，女24例。病理分级Ⅰ级26例，Ⅱ级22例，Ⅲ级32例，临床分期T1 20例、T2 20例、T3 22例、T4 18例。复发性膀胱癌46例。10例正常膀胱组织作对照。结果10例正常膀胱黏膜组织中RARβ均为强阳性。80例膀胱移行细胞癌中RARβ阳性表达28例，阳性率为35．0％。RARβ表达与膀胱移行细胞癌病理分级、临床分期以及复发密切相关（P〈0．05）。结论RARβ在膀胱移行细胞癌组织中存在表达缺失，其表达缺失程度与肿瘤病理分级、临床分期及复发相关，可作为观察膀胱移行细胞癌复发指标之一。

Expression of Retinoic Acid Receptor Beta in Bladder Transitional Cell Carcinoma and Its Clinical Significance

Objective To study the expression of retinoic acid receptor beta （RARβ） in bladder transitional cell carcinom a and its clinical significance .Medthods The expression of RARβ was exam ined in 80 cases （56 male and 24 female of the bladder transitional cell carcinom a and 10 cases of normal bladder tissue （controls ）by in m unohistochem istry medthod .The pathological grading showed Grade Ⅰ in 26 cases ,Grade Ⅱin22 and Grade Ⅲ in 32.The clinical staging showed T1 in 20 cases T2 in20,T3 in 22 and T4 in18.Of the 80 cases,46 developed relapse.Results The RARβ was strongly expressed in all the 10 cases of norm al bladder tissues with a positive rate of 100%;whereas the overall positive rate of RARβ in the 80 cases of blacder transitional cell carcinom a was 35.0%（28/80）.The positive rates of RARβ in Grade Ⅰ,Ⅱ,Ⅲ cases of bladder carcincm a were 76.9%（20/26）,22.7%（5/22）and 9.4%（3/32）respectively;the differences of positive rates between Grade Ⅰand Grade Ⅱ,Grade Ⅰand Grade Ⅲ cases was significant （P〈0.05）.The positive rates of RARβ in T1,T2,T3 and T4 cases of bladder transitional cell carcinom a were 75.0%（15/20）,45.0%（9/20）,13.6%（3/22）and 0.06%（1/18）,respectively ,which showed a trend of decrease in RARβ expression from T1 to T4 stage bladder transitional cell carcinom a.Result There was statistically significant difference between T1 and T3 or T4,and between T2 and T3 or T4stage bladder transitionalcell carcinom a （p〈0.05）The positive rate of RARβ in cases of bladder transitional cell carcinom a relapse was 17.4%（8/46）.Conclusions The RARβexpression is decreased in bladder transitional cell carcinoma;the degree of reduction is associated with pathologic grade and clinical stage of the cancer .It is suggested thet the RARβ may play an essential role in bladder carcinogenesis and can serve as a marker for prognosis of bladder cancer.
