经尿道前列腺汽化切割术对性功能影响的观察

目的 :了解经尿道前列腺汽化切割术 (TUEVAP)对性功能的影响。　方法 :参考英国激光泌尿协会尿道激光治疗良性前列腺增生 (BPH)随访表 ,设计性功能调查表包括性欲、阴茎勃起硬度、射精情况、勃起功能障碍(ED)。BPH病人汽化切割术后半年填表。　结果 :118例病人均填表完整 ,2 7 1% ( 3 3 / 118)病人性欲下降 ;3 1 7% ( 3 7/ 118)病人阴茎勃起硬度下降。 94例术前性功能正常的病人 ,术后发生逆行射精者占 3 5 1% ( 3 3 / 94 ) ;术后ED者占 8 5 % ( 8/ 94 )。　结果 :TUEVAP对性功能有不利影响 ,临床应予以重视。

The Effect of Transurethral Electrovaporization and Ablation of Prostate on Sexual Function

Objectives: To evaluate the effect of transurethral electrovaporization and ablation of prostate(TUEVAP) on sexual function in patients with benign prostatic hyperplasia(BPH). Methods: A questionnaire including four items was designed to assess the sexual changes in followed up patients with BPH before and after TUEVAP: sexuality (increase, nochange, decrease); rigidity (increase, no change, decrease); ejaculation (no change, retrograde ejaculation, painful ejaculation ); and erectile dysfunction (ED)(No, Yes). One hundred and eighteen patients selfadministered the questionnaire at 6 month follow up. Results: In 118 patients, 32(27.1%) were found with decreased sexuality after TUEVAP, and 37(31.7%) were found with decreased rigidity. In 94 patients who were potent before operation, 33(35.1%) were found with retrograde ejaculation after TUEVAP, and 8(8.5%)were found with ED. Conclusions: TUEVAP has an impact on sexual function in men with BPH, which must be taken into consideration in clinical practice.
