12例慢性髓性白血病髓外急性变的临床分析

目的总结慢性髓性白血病髓外急性变的临床特点，争取早期诊断、早期有效治疗。方法回顾性分析12例慢性髓性白血病髓外急性变的血常规及骨髓情况、髓外急性变的情况、诊治和预后情况，并与同期65例仅有骨髓急性变患者对比。结果慢性髓性白血病髓外急性变的确诊主要依赖于病变部位的病检；相比较于仅有骨髓急性变患者，外周血白细胞要低、血红蛋白及血小板要高，但生存期更短，有明显统计学差异，虽可化疗，但缓解率低，预后差。结论对于疑有髓外病变者，不论其血象及骨髓情况如何，均应尽量病检以尽早确诊；化疗效果差，有条件者可考虑酪氨酸激酶抑制剂治疗或异基因移植等治疗。

Clinical Analysis of 12 Cases of Chronic MyelogenousLeukemia with Extramedullary Blast Crisis

ObjectiveTo summarize the clinical features of chronic myeloid leukemia patients with extramedullary blast crisis for early diagnosis and effective treatment.MethodsThe characteristics of peripheral hemogram,and bone marrow,together with the diagnosis and some relevant clinical features of 12 chronic myeloid leukemia cases with extramedullary blast crisis were retrospectively analyzed.All of these data were comparatively analyzed with 65 cases of chronic myeloid leukemia without blast crisis in the same period.ResultThe diagnosis of chronic myeloid leukemia extramedullary blast crisis was mainly dependent on the pathological examination of the lesion site.Compared with the characteristics of only bone marrow BC cases,the peripheral white blood cells were lower,the hemoglobin and platelets were higher and the survival period was shorter for patients with extramedullary blast crisis.The difference is significant between the two groups.Although chemotherapy was administrated,but these patients were poorly relieved.ConclusionFor these suspected patients of extramedullary blast crisis,pathological examination should be performed for early diagnosis regardless of blood and bone marrow characteristics.The effect of chemotherapy was limited to these patients.Tyrosine kinase inhibitor and allogeneic transplantation were considerable options.
