胃癌穿孔的手术治疗

目的：探讨胃癌穿孔手术方式的合理选择。方法：回顾分析我院1992～2007年手术治疗胃癌穿孔36例。结果：有进展期胃癌症状者占83.33%（30/36）;胃癌穿孔直径平均2.1cm,组织病理学类型以低分化腺癌和印戒资料的细胞癌为主,占66.67%（24/36）。穿孔修补、胃大部切除和根治性切除手术后平均生存期分别为6、11和22.2个月。结论：胃癌穿孔是进展期胃癌的严重并发症,穿孔直径大,恶性程度高,预后差。尽量采用胃大部切除或根治性切除术可延长生存期。

Surgical therapy of perforation of gastric carcinoma

Objective：To investigate the reasonable surgical mode of perforation of gastric carcinoma.Methods：The clinical data of 36 cases with perforation of gastric carcinoma by surgical procedure from 1992 to 2007were analyzed retrospectively.Results：Thirty eases（88.33%） had the symptom with advanced gastric carcinoma; the mean diameter of perforation of gastric carcinoma was 2.1 cm ;24 cases （66.66%） were poorly differentiated adenoeareinoma and signet ring eeller carcinoma. The average survival months of patients treated with repair of perforation, palliative gastreetomy and radical gastreetomy was 6、11 and 22.2 months. Conclusion：The perforation of gastric carcinoma was the severe application of advanced gastric carcinoma.The larger of perforation diameter, Palliative gastreetomy or radical gastrectomy should be performed as soon as possible for prolong survival time.
