协同护理模式对慢性阻塞性肺疾病患者居家应用无创通气的影响

目的探讨应用协同护理模式对慢性阻塞性肺疾病患者在家中应用无创通气的干预方法及效果。方法将郑州市社医2010²011年出院的60例慢性阻塞性肺疾病稳定期患者,随机分为对照组和试验组,每组30例。第12011年出院的60例慢性阻塞性肺疾病稳定期患者,随机分为对照组和试验组,每组30例。第1²个月的健康宣教部分,试验组采用患者及家属参与的协同护理模式,对照组实施常规面授方式,第32个月的健康宣教部分,试验组采用患者及家属参与的协同护理模式,对照组实施常规面授方式,第3⁶个月的护理干预相同.比较两组6个月的急性发作次数、住院天数及生活质量。结果试验组急性发作次数、住院天数低于对照组(P<0.01),生活质量优于对照组(P<0.01)。结论应用协同护理模式能够提高慢性阻塞性肺疾病患者自我护理的知识和技能,提高无创通气的应用水平和患者依从性,降低慢阻肺急性加重的次数,提高生活质量。

Application of collaborative care model for chronic obstructive pulmonary disease patients with non-invasive mechanical ventilation

Objective To evaluate the effect of collaborative care model for chronic obstructive pulmonary disease （COPD） patients with non-invasive mechanical ventilation. Methods Sixty community-dwelling COPD patients with non-invasive mechanical ventilation were randomly divided into two groups,30 cases in each group. The patients in the control group received routine discharge guidance and follow-up education,while the patients and their family members in the experimental group participated in collaborative care model within two months after diseharge. The frequency of acute attacks,days of hospital stay and quality of life at 6 months after discharge were compared be- tween the two groups. Results Compared to the control group ,the frequency of acute attacks and days of hospital stay in the experimental group were significantly lower（P〈0.01）,and the score of quality of life was significantly higher（P〈0.01 ）. Conclusion The application of collaborative care model can improve COPD patients＇ self-care knowledge and skills,promote patient compliance on the application of non-invasive mechanical ventilation,reduce the frequency of acute attacks and improve the patients＇ quality of life.
