氟西汀合并小剂量奥氮平治疗抑郁症的对照研究

目的观察氟西汀合并小剂量奥氮平治疗抑郁症的临床疗效。方法70例抑郁症患者随机分成单用组和合用组,共观察8周,于治疗前、治疗后1,2,4,6,8周末采用汉密顿抑郁量表(HAMD),副反应量表(TESS)评定疗效及毒副反应。结果治疗第8周末,合用组与单用组的显效率分别为86%和68%,2者比较有显著性差异(P<0.05);合用组在1周末起效,单用组2周末起效;2组治疗后1周、2周、4周评分有显著性差异(P<0.05);2组的因子分在焦虑/躯体化和睡眠障碍方面有显著性差异(P<0.05)。结论氟西汀合用小剂量奥氮平治疗抑郁症起效快,克服了抗抑郁药本身起效慢的特点,可提高疗效,并能迅速改善睡眠障碍和焦虑/躯体化症状。

Control study of fluoxetine plus low dose olanzapine in the treatment of patients with depression

Objective It is to approach the effect of fluoxetine plus low dose olanzapine in treating the patients with depression.Methods 70 patients were randomly divided into control group(fluoxetine) and combined group(fluoxetine plus low dose olanzapine);the duration of study was 8 weeks.The efficacy and safety were assessed by Hamilton Depression Rating Scale(HAMD) and Treatment Emergent Symptoms Scale(TESS) at weeks 0,1,2,4,6 and 8.Results The therapeutic efficacy in combined group was significantly higher than that in control group.After 8 weeks,the improved rates were 88% in combined group and 67% in control group(P<0.05).Combined group act well earlier than control group by 1 week.The HAMD scores showed a significant difference at 1, 2 and 4 weeks after treatment(P<0.05).Besides,two sets showed an obvious difference in anxiety/somatization factor and sleeping disorder factor.Conclusion Fluoxetine plus low dose olanzapine has better effect and act early, may contribute to the delayed efficacy of fluoxetine,and improve sleeping and anxious,somatic symptoms quickly.
