脑钠素在老年人重度心力衰竭治疗前后的变化及其意义

目的 观察BNP水平与心脏超声指标的相关性;比较老年人重症心力衰竭患者短期治疗前后BNP的变化.方法 把所有受试者分为观察组及对照组.用免疫荧光标记法测定血浆BNP水平,用超声心动图测定左房内径（LA）、左室射血分数（LVEF）、左室舒张末期内径（LVEDD）,观察BNP水平与LA、LVEF、LVEDD的相关性.观察组短期正规治疗3天后复查心脏超声和BNP浓度,观察治疗前后各项指标的变化.结果 ①观察组的血浆BNP水平显著高于对照组,P＜0.01.②BNP值与LVEDD呈显著正相关,r=0.758,P＜0.01,与LVEF值（%）呈负相关,r=-0.779,P＜0.01.③短期治疗后BNP水平显著下降,P＜0.01,但LVEDD和LVEF无明显变化,P＞0.05.④住院期间死亡患者的BNP水平远远高于存活者,P＜0.01.结论 BNP测定是评价老年人重度心力衰竭治疗效果的有效方法.

Variation of BNP in the evaluation of the treatment of severe heart failure in senile patients

Objective To observe the correlation between BNP level and cardiac ultrasonic index, compare the BNP levels before and after a short-term treatment of severe heart failure in senile patients. Methods All subjects were randomized into two groups: the observation group and the control group. Observed the correlation between plasma BNP level (determined by immunofluorescence method) and LA, LVEF, LVEDD (determined by ultrasonography). After a 3-day normal treatment, reexamined the cardiac ultrasound and BNP level of the observation group to observe the changes of all indexes. Results {1} Plasma BNP level of the observation group was significantly higher than that of the control group, P<0.01. {2} BNP level had positive correlation with LVEDD, r=0.758, P<0.01, negative correlation with LVEF, r=-0.779, P<0.01. {3}BNP level was significantly decreased after the short-term treatment, P<0.01, but no significant change of LVEDD and LVEF, P>0.05. {4} BNP level of the died patients was significantly higher than that of the survivals, P<0. 01. Conclusion BNP determination is effective in the evaluation of the treatment of severe heart failure in senile individuals.
