肾核素动态显影在小儿先天性肾盂输尿管连接部梗阻中的应用

摘    要:目的总结肾核素动态显影在小儿先天性肾盂输尿管连接部梗阻中的应用。方法2000年1月~2006年1月我院共收治52例先天性肾盂输尿管连接部梗阻、肾积水患儿,采用99Tcm-DTPA肾动态显影对52例患儿进行检查,测定肾小球滤过率(GFR),并对21例进行手术前后GFR比较。结果肾影像及肾图曲线随病情不同而出现特征性表现,GFR随病情的加重而出现显著性下降(P<0.01),术后GFR升高,与术前比较,差异有显著统计学意义(P<0.01)。结论99Tcm-DRPA肾动态显影可作为诊断小儿先天性肾盂输尿管连接部梗阻、术后病情观察的有效检查方法。

Abstract:Objective To evaluate the clinical value to renal radionuclein dynamic imaging in pediatric hydronephrosis due to anomalies of pyeloureteral junction.Methods 52 unilateral obstruction of pelviureteric junction were studied by 99Tcm-DTPA renal dynamic imaging and glomerular filtration rate(GFR) quantitative analysis was performed,21 of them underwent surgical treatment and underwent the dynamic imaging pre-and post-operation.Results Renal image and nephrogram were difference characteristically according to different clinical manifestation.And positive correlation was discovered between glomerular filtration rate(GFR) and clinical situation.(P< 0.01),GFR increased significantly post-operation,compared with pre-operatino(P < 0.01).Conclusion 99Tcm-DTPA ranal dynamic imaging can offer references for diagnosis,assessment of patient's condition and therapeutic efficiency for those with pediatric hydroneparosis due to anomalies of pyeloureteral junction.
