细管胸腔闭式引流治疗恶性胸水临床观察（附14例分析）

目的：探讨细管胸腔闭引流术与VP16、红霉素治疗恶性胸腔积液的临床疗效。方法：对有临床、病理依据的恶性胸腔积液患，经X片、B超胸水定位后置16＃锁骨下静脉穿刺留置针入胸腔，积液引流干净后予胸腔内注射胸膜硬化剂VP16、红霉素。结果：14例恶性胸腔积液患显效6例、有效5例、无效3例。无效均为肺癌复治患。结论：该法治疗恶性胸腔积液具有患痛苦少、疗效确切及不易复发等优点，便于临床应用。

Clinical observation of malignant pleural effusion treating with small catheter for closing drainage in fourteen patients

Objective: To evaluate the clinical effects of malignant pleural effusions in treatment using small catheter for closing drainage and VP16 and erythromycin for intrapleural injection. Methods: The patients were all malignant pleural fluid proved patho-logicall. The intrapleural installation of 16 small catheter was used for fluid drainage, after location of fluid level by X-ray films and ultrasound scan. The sclerosing agents were Vpl6 and erythromacin injected intrapleurally after the pleural fluids were adequate evacuated. Results; The 14 patients were evaluable for response. The complete, partial, no response were in 6, 5, 3 cases respectively. The non-responders were all retreated malignant effusions.Conclusions: This approach has a series of advantages, such as less painful, fayorable effects, lessed retreated necessarily and useful clinically.
