硬膜外注射治疗腰椎间盘突出症的诱发电位研究

目的观察硬膜外注射对腰椎间盘突出症的疗效,探讨体感诱发电位(SEP)在病情及疗效评估中的作用。方法将52例腰椎间盘突出患者随机分为硬膜外注射组26例和常规治疗组26例,治疗前及治疗1个月后行下肢SEP检查并对症状、体征进行临床评定,对照分析硬膜外注射的疗效及对神经电生理的影响。结果硬膜外注射组疗效显著,优良率为88.4%。治疗前SEP异常率为73.1%,SEP异常主要表现为:潜伏期延长及神经传导速度减慢。硬膜外注射治疗可改善马尾电位的潜伏期及神经传导速度,较常规治疗方法差异有显著性意义(P<0.05)。结论硬膜外注射对腰椎间盘突出症有较好的临床疗效,SEP可作为腰椎间盘突出症病情及疗效评定的指标。

The study of evoked potentials in peridural injection on lumbar disc herniation

Objective To observe the therapeutic effect of peridural injection on lumbar disc h erniation and to explore the so-matosensory evoked potential on the assessment of the illness.Methods 52cases of lumbar disc herniation we re divided into the peridural injection group 26cases and the trad itional group 26cases.The low limb s omatosensory evoked potential was e xamined and clinical symptoms and signs were assessed before and after 1month of treatment.The treatment effect and effect on neur oelectrophysiology of peridural injection were analysed.Results The peridural injection had markedl y curative effect.The eminent rates were 88.4%.The ab-normal rate of SEP before treatment was 73.1%.The main abnormity were delayed latent period and decreased nerve transmit velocity.Compared with traditional treatmen t,the peridural injection can impro ve markedly the CE latent period and n erve transmit veloci-ty.Conclusions The peridural injection has prefera ble clinical curative effect.The so matosensory evoked potential can be an index of curative effect of acute lumbar disc herniation.
