喉癌围术期肠内营养支持的临床应用

摘    要:目的探讨喉癌围术期采用不同的营养支持方法对喉癌患者术后恢复的影响。方法将84例喉癌术后患者按手术时间顺序间隔随机分为对照组、实验A组、实验B组。对照组28例,术前正常进餐,术后6h鼻饲高能匀浆膳肠内营养;实验A组28例,术前正常进餐,不间断地添加普通肠内营养混悬液,术后6h鼻饲高能匀浆膳肠内营养;实验B组28例,术前正常进餐,不间断地添加肠内免疫营养混悬液,术后6h鼻饲高能匀浆膳肠内营养。比较3组患者术后的免疫功能指标、营养指标、术后咽瘘发生率、切口Ⅰ期愈合率及平均住院时间。结果实验A组和实验B组术后第9天的LgG、LgM、LgA、CD3^+、CD4^+、CD4^+/CD8^+、BW、PA、ALB与对照组术后第9天比较,差异有统计学意义(P〈0.05);实验B组术后第9天的LgG、LgM、LgA、CD4^+、CD4^+/CD8^+与实验A组比较,差异有统计学意义(P〈0.05)。实验A组、B组的术后咽瘘发生率、切口Ⅰ期愈合率及平均住院时间与对照组比较,差异有统计学意义(P〈0.05)。结论喉癌患者进行围术期肠内免疫营养支持,能提高机体免疫功能,纠正营养不良,降低术后并发症,提高喉癌患者的生存质量。
