内镜下鼻窦手术167例临床分析

目的 探讨鼻内镜下手术治疗慢性鼻窦炎、鼻息肉的疗效。方法 对1995－1999年间实行内镜下鼻窦手术并完成术后随访的167例慢性鼻窦炎、鼻息肉患者进行临床分析，其中1型58例（34．73％），2型97例（58．08％），3型12例（7．19％）。105例局部麻醉＋静脉强化麻醉下完成，62例在全身麻醉下完成，同期行鼻中隔矫正术28例，术后随访3－12个月。结果 1型治愈率94．83％，2型治愈率76．29％，3型治愈率50．00％。三型之间治愈率有显著性差异（P＜0．01）。手术并发症5例，发生率2．99％。结论 鼻内镜下手术是治疗慢性鼻窦炎、鼻息肉的最有效方法，手术疗效与临床分型及术后治疗密切相关。

Clincal analysis of 167 cases of endoscopic nasal sinus operat ion

Objective To study the effect of endoscopic sinus (ESS) in the treatment of chronic sinusitis and nasal polyps.Methods To analyze 167 cases with chronic sinusitis and nasal polyps,who underwent ESS and were followed up.There were 58 cases (34.73%) in type 1,97 cases (58.08) in type 2 and 12 cases (7.19%) in type 3.105 cases underwent ESS under intranasal local anesthesia and sedation;62 cases underwent ESS under intranasal general anesthesia;28 cases underwent submucous resection of nasal septum or septoplasty long time follow up was performed srom 3 to 12 months.Results The cured rate was 94.83% in type 1,76.92% in type 2 and 50.00% in type 3.There was significant difference in cured rate among different types ( P <0.01).The complication of ESS was found in 5 cases (2.99%).Conclusions ESS is effective in the treatment of nasal polyps and chronic sinusitis.The cured result is directly related to the clinical types and the treatment after operation.
