尺骨骨折术后医源性骨不连原因分析

目的认识尺骨骨折术后医源性骨不连的发生原因.方法对38例尺骨骨折骨不连患者的X线片、骨折的类型、手术方式及内固定材料等综合分析来判断导致骨不连的原因.结果尺骨骨折术后医源性骨不连原因较多,包括对内固定物选择不当,对内固定操作不规范,缺乏对骨折治疗"生物学固定"原则的理解,忽视必要植骨,缺乏正确的康复锻炼和医源性感染.结论尺骨骨折治疗应遵循骨折固定的基本原则、必要植骨、术后正确的康复训练和避免感染是预防医源性骨不连的关键.

To investigate the cause of iatrogenic nonunion of fracture of diaphysis of ulna

Objective To recognize the causes of iatrogenic nonunion of fracture of diaphysis of ulna .Methods Thirty-eighy nonunion cases were reviewed and synthetically analyzed in the X-ray films ,type of the fractures,techniques of operation and interfixators.Results Many factors may lead to iatrogenic nonunion of frature of diaphysis of ulna ,including improper interfixators ,incorrect operative techniques,being short of the principle of biological fixation and ignoring bone transplantation, lack of correct guidance of post operative habilitation and iatrogenic infection.Conclusion The keys to prevent the iatrogenic nonunion are to follow the principle of internal fixation in early management of fractures,transplant bone,giving the correct guidance of post operative rehabilitation and preventing iatrogenic infection.
