慢性乙型肝炎患者的情绪障碍调查及治疗对策

对慢性乙型肝炎患者情绪障碍发生情况进行调查并探讨治疗对策。采用汉密尔顿抑郁量表（HAMD）和焦虑量表（HAMA）及症状自评量表（SCL-90）对261例慢性乙型肝炎患者进行评定，并对部分情绪障碍患者给予盐酸氟西汀20mg/d及一定心理治疗4～8周。结果261例慢性乙肝患者中有81例患者有情绪障碍，占31.0%。情绪障碍发生率与乙肝病情轻重无明显关系。抗抑郁治疗能明显改善患者的情绪障碍，并能提高乙型肝炎总的治愈率，同时缩短住院时间。结果表明慢性乙肝患者存在一定比例情绪障碍，抗抑郁药物及心理治疗对病情恢复有帮助。

The Survey of Psychiatric Disorders among the Patients withChronic Viral Hepatitis and Treatment Measures

The prevalence of psychiatric disorders among the patients with chronic viral hepatitis and the effective treatment measures were investigated. 261 patients with chronic viral hepatitis were assessed by Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA) and Symptom Check List 90 (SCL 90). A part of patients with depressive disorders were treated with fluoxetine, 20 mg per day for 4 to 8 weeks. The results showed that 81 out of 261 patients suffered from psychiatric disorder, accounting for 31.0 %. The occurrence rate of this disorder had no relationship with the severity of hepatitis. The administration of fluoxetine showed better effect on depressive disorders and could raise cure rate and shortened the duration of hospital stay. It was indicated that a proportion of patients with chronic viral hepatitis suffered from psychiatric disorders and fluoxetine and mental treatment showed better effects.
