肺癌患者外周血癌胚抗原 mRNA和细胞角蛋白19 mRNA的联合检测

摘    要:目的 观察肺癌患者外周血循环细胞中癌胚抗原 (CEA)mRNA和细胞角蛋白 19(CK19)mRNA的表达情况 ,并探讨CEAmRNA和CK19mRNA联合检测的临床意义。方法 用RT PCR方法 ,分别检测 5 0例肺癌患者、2 5例肺部良性疾病患者和 37例健康志愿者外周血中CEAmRNA和CK19mRNA的表达 ,并对CEAmRNA和CK19mRNA联合检测与单项检测的阳性率进行比较 ,分析联合检测对肺癌诊断的灵敏度与特异性。结果  5 0例肺癌患者中 ,CEAmRNA阳性表达率为 6 0 .0 % (30 / 5 0 ) ,CK19mRNA阳性表达率为 38.0 % (19/ 5 0 ) ,两者联合检测阳性率为 82 .0 % (41/ 5 0 )。联合检测与单项检测相比 ,能显著提高诊断灵敏度 ,且并不影响特异性。联合检测与单项检测相比 ,灵敏度差异有显著性 (P <0 .0 5 ) ,特异度差异无显著性 (P >0 .0 5 )。同时 ,肺癌患者的联合检测阳性率亦均显著高于肺部良性疾病组 (2 5例 ,阳性率 2 8.0 % )和健康对照组 (37例 ,阳性率 8.1% ;P <0 .0 0 1)。结论 外周血循环细胞中CEAmRNA和CK19mRNA的联合检测可提高肺癌细胞向外周血播散的检出率 ;随访并扩大样本含量有助于探讨其在检测微转移中的意义

Abstract:Objective To investigate the expression of CEA mRNA and CK 19 mRNA in peripheral blood cells of patients with lung cancer and evaluate its clinical significance. Methods Peripheral blood nucleated cells of 50 patients with lung cancer were studied by RT PCR to detect the expression of CEA mRNA and CK 19 mRNA. Results The combined positive rate of CEA mRNA and CK 19 mRNA in patients with lung cancer (82.0%) was significantly higher than that in patients with benign lung diseases(28.0%) and healthy volunteers(8.1%) ( P <0.001). The expression rate had no relation to the clinical staging or histological type. Compared with single detection, combined detection increased the detection rate but did not decrease the specificity. ConclusionCombined detection of CEA mRNA and CK 19 mRNA expression in peripheral blood nucleated cells increase the sensitivity of detecting hematogenous dissemination of cancer cells. Long term survival analysis and more specimens would be helpful for evaluating its clinical significance.
