动态人体重心平衡训练联合双跑带运动平板训练对老年脑卒中患者运动功能的影响分析

摘    要:目的 探讨动态人体重心训练联合双跑带运动平板对老年脑卒中患者运动功能的影响。方法 选择2015 年1 月~2017 年1 月期间我院收治的106 例老年脑卒中患者,将所有患者随机平均分为研究组53 例和对照组53 例,所有患者入院后在进行常规药物治疗的同时采用不同康复训练模式,对照组患者给予常规康复训练,研究组患者在常规康复训练的同时,取消步行训练,采用双跑带运动平板训练,平衡训练在动态人体重心监测系统下以坐-立转移的方式进行。所有患者均于入组前及训练4 周后进行运动功能评价,通过Berg 平衡量表(BBS)对患者的平衡能力进行评价;采用Fugl-Meyer 功能评分量表(FMA)对患者的下肢功能进行评价;采用功能性步行能力量表(FAC)对患者训练后的步行能力进行评价。同时采用Barthel 指数(BI)对患者训练前及训练后2 周、训练后4 周的日常生活能力进行评价。结果 两组患者经过训练后平衡能力及下肢功能均有显著改善,研究组患者训练后的BBS 评分及FMA 评分均显著高于对照组,差异均具有统计学意义(P<0.05),同时训练后研究组患者的步行能力显著优于对照组,差异具有统计学意义(Z=4.106,P<0.05);两组患者经过训练日常生活能力显著提升,研究组患者训练2 周及4 周后的BI 评分均显著高于对照组,差异均具有统计学意义(P<0.05)。结论 动态人体重心训练联合双跑带运动平板能显著改善老年脑卒中患者的平衡能力、运动能力,对提高患者的预后及日常生活能力效果优于常规康复训练。

Effect of dynamic testing of the gravity center of human body combined with double running belt treadmill on post-stroke motor function in elderly patients
Abstract:Objective To investigate the effect of dynamic testing of the gravity center of human body combined with double running belt treadmill on motor function in elderly stroke patients. Methods A total of 106 elderly stroke patients admitted in our hospital from January 2015 and January 2017 were randomly divided equally into 53 patients in the study group and 53 in the control group. All patients received routine drug treatment as well as different rehabilitation training modes after admission. Patients in the control group were given conventional rehabilitation training. Patients in the study group were treated with double running belt treadmill in the course of routine rehabilitation training instead of walking training. And balanced training was performed by the way of sitting and standing transfer under the dynamic human gravity center monitoring system. All patients were evaluated for motor function before admission and after 4 weeks of training. The balance ability of the patients was assessed by the Berg Balance Scale(BBS). The function of the lower extremities was assessed using the Fugl-Meyer Functional Rating Scale(FMA). The patient''s walking ability after training was evaluated using the Functional Walkability Scale(FAC). At the same time, the Barthel index(BI) was used to evaluate the ability of daily living before training, at 2 weeks and 4 weeks after training. Results After training,the balance ability and lower extremity function in both groups significantly were improved. The BBS score and FMA score after training in the study group were significantly higher than those in the control group, and the difference was statistically significant (P<0.05). The walking ability of the study group was significantly better than that of the control group after training, and the difference was statistically significant (Z=4.106, P<0.05). The ability of daily living was significantly improved in the two groups after exercising. The BI score in the study group was significantly higher than that of the control group after being trained for 2 weeks and 4 weeks, and the differences were statistically significant(P<0.05). Conclusion Dynamic testing of the gravity center of human body combined with double running belt treadmill exercise can significantly improve the balance ability and exercise capacity of elderly stroke patients, and it has a better effect than routine rehabilitation training in improving the prognosis and daily living ability of patients.
