口服小剂量美沙酮治疗老年慢性癌痛患者的临床观察

目的 :观察口服小剂量美沙酮对老年慢性癌痛患者的镇痛效果和不良反应。方法 :给予老年重度癌痛患者美沙酮 2 0 - 4 0mg·d- 1 ,分 2次口服 ;于治疗前后对患者进行疼痛强度和生活质量评估 ;观察不良反应 ,并对症治疗。结果 :口服小剂量美沙酮前后疼痛强度 (数字分级法NRS)对比差异有极显著性 (P <0 0 0 1) ;生活质量部分相关项目如精神、睡眠、疲乏、疼痛、日常生活、面部表情的评估得分对比差异有极显著性 (P <0 0 0 1) ;不良反应轻微。结论 :小剂量美沙酮口服治疗老年慢性癌痛患者疼痛效果显著 ,患者生活质量得到明显改善 ,不良反应轻微。

CLINICAL OBSERVATION ON THE TREATMENT WITH SMALL- DOSE METHADONE FOR CHRONIC PAIN OF AGED CANCER PATIENTS

Objective: To observe the analgesic effect and adverse reactions of small-dose methadone for chronic pain of aged cancer patients. Methods: The patients were taken orally methadone 20-40 mg per day in two separate doses. Before and after this treatment, the intensity of pain and the quality of life(QOL) were evaluated, and the adverse reations were observed. Results: There were extreme significant differences(P<0.001) in the intensity of pain after using methadone. There were also extreme significant differences (P<0.001) in the items of quality of life such as vigour, sleep, weariness, pain, daily life and facial expression after this management. The adverse reactions were slight. Conclusion: The analgesic effect of small-dose methadone for chronic pain of aged cancer patients is significant with slight adverse reactions. The quality of life of the patients is improved.
