老年慢性阻塞性肺疾病氨茶碱中毒75例分析

目的　探讨老年慢性阻塞性肺疾病 (COPD)患者使用氨茶碱治疗过程中出现的中毒反应及影响因素。方法　回顾性分析 75例COPD病例的临床资料 ,对比分析不同病程、不同动脉血气PaO2 、PaCO2 及有无合并症等情况下患者氨茶碱中毒的发生率。同时总结COPD患者氨茶碱中毒的临床表现。结果　不同病程 ,不同动脉血气PaO2 和PaCO2 ,有、无合并症的者氨茶碱中毒发生率间差别均有显著性意义 (P <0 0 1)。中毒反应主要表现为 :心律失常、神经系统症状、消化系统症状或原有呼吸系统症状加重。结论　COPD患者并发肺心病、冠心病、肝功能不全、PaO2<6 0mmHg时 ,使用氨茶碱治疗易发生中毒反应 ,此类患者使用氨茶碱剂量不宜过大

Analysis of aminophylline intoxication on 75 old patients with chronic obstructive pulmonary disease

Objective To explore the clinic manifestation and influence factor of aminophylline intoxication during COPD patients treated with aminophylline.Methods Clinic date were analyzed by retrospectively methods.The occurring rates were contrasted between course of disease>15yrs and <15yrs;PaCO 2>60mmHg and PaCO 2<60mmHg;company with chronic pulmonary disease,coronary atherosclerotic heart disease,hypohepatia and company with none of them.The manifestations of aminophylline intoxication on COPD patients were summed up in the same time.Results Significant differences were found between the occurring rates of aminophylline intoxication of course of disease >15yrs and <15yrs;PaCO 2>60mmHg and PaCO 2<60mmHg;patients having complication and not having complication.The manifestations of aminophylline intoxication include cardiac arrhythmias,nervous system symptoms,gastrointestinal system symptoms or aggravation of original respiratory system symptoms.Conclusion COPD patients easily experimented aminophylline intoxication when they were treated with aminophylline if they complicate chronic pulmonary heart disease,or coronary atherosclerotic heart disease,or hypohepatia,or PaCO 2<60mmHg in arterial blood gas analysis.So,the dose of aminophylline should be decreased on these patients in contrast with the patients who have not complication.
