经翼点入路显微外科治疗大型垂体腺瘤

目的报道经翼点入路或其改良入路显微手术切除18例垂体腺瘤并探讨优、缺点.方法经翼点开颅,采用显微手术解剖鞍区各脑池,充分利用鞍区4个解剖间隙切除肿瘤.结果18例中全切除或次全切除14例,无手术死亡,无严重并发症,复发2例.结论采用改良翼点入路显微手术切除大型垂体腺瘤,疗效满意.

Microsurgical resection of large pituitary adenoma via pterional approach

Objective:To introduce the experiences of large p ituitary adenoma treated by improved microsurgical opreation via pterional appro ach,discuss the aduantages and disaduantages of it. Methods:18 c ases with large pituitary adenoma ,which accepted microsurgical operation via im proved piterional approach,were reviewed retrospectively.The cisterns of the se llar region was dissected first under the operative microscope and the microsurg ical resections were then done by making use of the fully exposed anatomic space .Results:In the series of patients ,total removal of the tumor w as achieved in 14,with no death of the operation or severe complication .Two pat ients relapsed during follow-up period .Conclusions:The authors considered that satisfactory outcome could be expected when the reformed piterio nal approach was used for microsurgical operation on the patient with large pitu itary adenoma.
