辅酶 Q_(10)对心肌缺血再灌注损伤的保护作用

摘    要:观察辅酶Q10对心肌缺血再灌注损伤的保护作用,随机选择20例拟行体外循环心内直视手术的患儿分为A组:辅酶Q10组,B组:对照组,各10例。在术前6天、术晨、阻断升主动脉前5分钟、开升主钳后15分钟和30分钟,术后第1天和第4天的7个时点抽取股静脉血测丙二醛、磷酸肌酸激酶同工酶、α-羟丁酸脱氢酶和内源性超氧化物歧化酶的血清浓度。结果:心肌缺血再灌注后血清丙二醛、磷酸肌酸激酶同工酶、α-羟丁酸脱氢酶浓度均显著增加,而内源性超氧化物歧化酶浓度显著下降,术后变化也很显著,两组间各时点术后变化有显著差异,A组变化较B组轻,恢复也较快。说明氧自由基对心肌缺血再灌注损伤的作用至少可持续到术后3天或更长时间,辅酶Q10对减轻氧自由基的损害非常有效,应从整个围术期考虑应用。

Abstract:In order to observe the protective role of CoQ 10 to myocardial ischemia reperfusion injury,20 patients who would be given intracardiac surgery under extracorporeal circulation were selected at random and divided into two groups (A group:Co Q 10 ,B group:control).According to our design,at the seven time points (the 6th day before operation,the morning of preoperation,5 minutes before clipping aorta,15 and 30 minutes at loosing the clip,the lst and 4th days after operation),MDA content in serum,the activities of CPK MB,α HBDH and SOD were determined with taking blood from femoral vein.Results:We found that their MDA content in serum was all increased,the activities of CPK MB and α HBDH were greater than their preoperative values,but the activity of SOD was lower than their ones before ischemia in Group A (Group:Co Q 10 ) and Group B (Group:control).The changes at each time point varied with the two groups;A group′s change was milder than B group′s,and the recovery of A group got quicker than B group.It′s concluded that the role of free oxygen radical to myocardial ischemia reperfusion injury may last at least,3 days and perhaps more time,but Co Q 10 can alleviate the damage.
