磁共振质子波谱分析在精神分裂症患者前额叶研究中的应用

摘    要:目的:研究精神分裂症患者前额叶磁共振质子波谱(1H-MRS)特点。方法:对34例7天内未服用抗精神病药物的精神分裂症患者和34名正常人在入组24h内分别进行多体素1H-MRS检查(TR1500ms,TE135ms),所测定的代谢产物包括N-乙酰基天门冬氨酸(NAA)、肌酸复合物(Cr)、胆碱复合物(Cho),系统自动完成比值计算。对精神分裂症患者同时进行PANSS评分。结果:精神分裂症患者左侧前额叶NAA/Cr值显著低于对照组(1.31±0.24/1.39±0.26,t=2.453,P〈0.05);且左侧NAA/Cr的水平与阳性和阴性症状量表(PANSS)总分及阴性症状分呈负相关(r=-0.365,P〈0.05;r=-0.416,P〈0.05)。结论:精神分裂症患者左侧前额叶神经元功能低下。

Abstract:Objective:To identify the metabolite levels in prefrontal lobe in patients with schizophrenia by proton magnetic resonance spectroscopy (1 H-MIlS). Methods:34 sclfizophrenies with medicine-free for at least 7 days and 34 normal controls were assessed in tiffs study. A muhi-voxel H-MRS provided by Siemens Medical Ine was given to all the subjects within 24 hours they got in hospital. The NAA, Cho and Cr were measured and the ratios of NAA/Cr, Cho/Cr and NAA/(Cho + Cr) were determined. The patients were also assessed by means of positive and negative symptom scale( PANSS). Results: Relative to the comparison subjects, the patients demonstrated significantly lower NAA/Cr ratio in left prefrontal lobe ( 1.31 ± 0,24/1.39 ± 0.26, t = 2.453, P 〈 0.05 ). The NAA/Cr ratio in left prefrontal lobe was significantly negatively correlated with the total score and the negative symptoms score of PANSS respectively ( r = - 0.365, -0.416,all P 〈 0.05). Conclusion: Damage and loss of neurons in left prefrontal lobe was suspected in schizophrenics with medicine-free for at least 7 days.
