复发性脑胶质瘤的数字减影脑血管造影表现及其意义

目的:研究分析复发性脑胶质瘤的数字减影脑血管造影(DSA)表现,并结合对复发性脑胶质瘤行超选择性插管灌注化疗的疗效,探讨DSA表现分型的临床意义.方法:对49例复发性脑胶质瘤行超选择性脑动脉插管DSA检查,并根据肿瘤的供血动脉,肿瘤染色和回流静脉对复发性脑胶质瘤的DSA表现进行分型.结果:复发性脑胶质瘤的DSA表现可分为4型,其表现主要与肿瘤的恶性程度、生长速度以及手术切除的范围有关.结论:复发性脑胶质瘤由于多经历了手术、放疗和化疗,其DSA表现与原发病灶有所不同,经肿瘤供血动脉或肿瘤所在部位的脑动脉行超选择性插管化疗,可明显提高复发性脑胶质瘤介入化疗的疗效,降低化疗药物的毒性反应.

DSA appearance of recurrent cerebral glioma and its significance

To investigate the digital substract an-giography (DSA) findings of 49 patients withrecurrent cerebral glioma. The significance of DSA appearances and the result of supersclcctive catheterized intracerebral arterial infusion of chemotherapy concerned were also analysed. Method: The DSA via superselective intracerebral arteries catheterization was performed in 49 patients with recurrent glioma, and its vascular pattern was classified acorrding to the feeding arteries, tumor stain and its draining veins of the tumor mass. Result: DSA appearance of recurrent glioma may be classified to 4 types which correlated with the tumor' s malignancy, growth rate and the extent of surgical resection. Conclusion: Owing to the recurrent glioma had undergone surgery, radiotherapy and/or chemotherapy, its DSA appearance was different from that of the original one. Superselective intra arterial infusion chemotherapy was proved to be an effective method for the treatment of recurrent glioma.
