不同促排卵方案治疗多囊卵巢综合征并不孕症临床研究

目的探讨不同促排卵方案治疗多囊卵巢综合征(PCOS)并不孕症的临床疗效。方法选取南通大学附属海安医院2016年10月至2019年6月收治的PCOS并不孕症患者135例,随机分为3组,各45例。观察组患者予枸橼酸氯米芬片、来曲唑片、注射用尿促性素和注射用绒促性素治疗;对照1组患者不予来曲唑片、对照2组患者不予枸橼酸氯米芬片,两个对照组其余治疗措施同观察组。结果首个治疗周期结束时,与两个对照组比较,观察组患者的雌二醇(E2)水平、子宫内膜厚度、宫颈黏液评分及临床妊娠率均显著升高,成熟卵泡数显著增多,最大卵泡直径显著增大(P <0.05),睾酮(T)水平、黄体生成素/卵泡刺激素(LH/FSH)值、卵巢过度刺激综合征(OHSS)发生率和早期流产率均显著降低(P <0.05);三组的不良反应发生率相当(P <0.05)。结论枸橼酸氯米芬联合来曲唑可调节PCOS并不孕症患者的性激素水平,提高促排卵效果,改善妊娠结局。

Clinical Study of Different Ovulation Induction Schemes in the Treatment of Patients with Polycystic Ovary Syndrome Complicated with Infertility

Objective To investigate the clinical efficacy of different ovulation induction schemes in the treatment of patients with polycystic ovary syndrome(PCOS)complicated with infertility.Methods A total of 135 patients with PCOS complicated with infertility admitted to the Hai’an Hospital Affiliated to Nantong University from October 2016 to June 2019 were selected and randomly divided into three groups,45 cases in each group.The patients in the observation group were treated with Clomiphene Citrate Tablets,Letrozole Tablets,Urotropin for Injection and Chorionic Gonadotropin for Injection,the patients in the control groupⅠreceived the same therapy as the treatment group without taking Letrozole Tablets,the patients in the control groupⅡreceived the same therapy as the treatment group without taking Clomiphene Citrate Tablets.Results At the end of the first treatment cycle,compared with those in the two control groups,the estradiol(E2)level,endometrial thickness,cervical mucus score and clinical pregnancy rate in the observation group were significantly higher,the number of mature follicles in the observation group was significantly more,the diameter of the largest follicle in the observation group was significantly longer,while the testosterone(T)level,the luteinizing hormone/follicle-stimulating hormone(LH/FSH)value,the incidence of ovarian hyperstimulation syndrome(OHSS)and the early abortion rate in the observation group were significantly lower(P<0.05).The incidence of adverse reactions in the three groups was similar(P<0.05).Conclusion Clomiphene citrate combined with letrozole can regulate the sex hormone level of patients with PCOS complicated with infertility,improve ovulation induction effect and pregnancy outcome.
