隐性腭裂的手术方法探讨

目的探讨隐性腭裂的手术治疗方法。方法本组病例采用两种术式,术式一:切除软腭正中隐裂区粘膜,显露两侧肌层,松解后拉拢缝合。术式二:切除隐裂区粘膜,单瓣后推腭粘膜骨膜瓣,腭帆提肌吊带重建,软腭鼻腔侧粘膜Z成形,同步延长软腭,封闭裂隙。结果15例患儿,隐裂均得以修复,发音改善。结论术式一操作简便,损伤小,适用于幼儿或隐裂窄小的患者;术式二恢复软腭的正常解剖及生理结构,明显改善腭咽闭合功能。

Explore of Operative Methods for Recessive Cleft Palate

Objective To explore the operation methods for recessive cleft palate.Methods The first method: Excising the midline mucous membrane of the soft palate,loosing and stitching the both sides of mucous membrane and muscle.The second: excising the midline mucous membrane,pushing back the mucosal flap of the hard palate,reconstructing the sling of the levator veli palatini muscle and making Z-plasty on the mucosa near nasal cavity.Results All of the 15 cases obtained satisfactory results,their pronunciation improved obviously.Conclusions The first method fits for the younger children or the children with narrow recessive cleft palate;Because of recovering the anatomic length and function of soft palate,so the second method is worth of further application.
