液基细胞学联合阴道镜检查对宫颈疾病中的诊断价值

目的研究宫颈液基细胞学检查（TCT）联合阴道镜检查对宫颈疾病筛查的意义及临床价值，并且对于联合诊断的优点进行了分析。方法选择我院2009年10月到2011年10月两年内在我院就诊的1000例妇女行TCT、阴道镜检测和宫颈活检组织学检查，比较TCT对宫颈细胞异常的检出率以及TCT检测联合阴道镜对H宫颈疾病的检出率，以病理学诊断为金标准。结果TCT的检出率为21％，TCT联合阴道镜的总检出率为37．0％，组织学阳性病例检出率41．9％，组织学阴性病例检出率4．5％，两者联合检测率高于TCT单独检出率，呈极其显著性差异，P〈0．01，并且以病理学为金标准的基础，联合检查的检出率更加符合病理学结果，P〉0．05。结论宫颈液基细胞学检查（TCT）联合阴道镜检查是筛查早期宫颈病变的最佳选择，能明显提高诊断的准确性和早发现癌前病变。

The diagnostic value of TCT combined with colposcopy in treating cervical diseases

Objective This paper is to study the significance and clinical value of TCT combined with colposcopy for screening the cervical diseases, and to analyze the advantages of the combined diagnosis method. Method 1000women who had been treated in our hospital from Oct, 2009 to Oct, 2011 were selected to undergo TCT, colposcopy and cervical biopsy, and the detection rate of abnormal cervical cells with TCT and the detection rate with TCT combined with colposcopy are compared, with the histopathologic diagnosis as the golden standard. Results The detection rate with TCT is 21%, and with TCT combined with calposcopy is 37.0%; the detection rate of positive cases through histological examination is 41.9%, and the detection rate of negative cases through histological examination is 4.5%; the detection rate through the two combined diagnosis method is higher than that of exclusive TCT, which presents significant difference（P〈0.01）; besides, for the golden standard, based on pathology, the detection rate with combined diagnosis method is more accordant with the pathology results（P〉0.05）. Conclusions It is a best choice to screen early cervical diseases with TCT combined with colposcopy, which can obviously improve the accuracy of diagnosis and early discover the precancerous lesions time.
