输卵管间质部妊娠两种手术方法的临床疗效比较

[目的]探讨腹腔镜下输卵管间质部妊娠手术的适应证及疗效.[方法]分析40例输卵管间质部妊娠病例资料,其中18例行腹腔镜手术,22例行开腹手术,对两种手术患者的情况进行比较.[结果]两组患者手术时间、血β-HCG术后24 h下降程度、血β-HCG转阴天数相比差异无显著性（P〉0.05）,而住院日、保留导尿管天数、肛门排气天数、术后镇痛剂、住院费用相比差异有显著性（P〈0.05）.[结论]腹腔镜下输卵管间质部妊娠手术与开腹手术相比,具有明显优越性,创伤小,恢复快,安全可行,值得推广.

Clinical Analysis of the Operation for 40 Cases of Interstitial Tubal Pregnancy

[Objective]To explore the clinical indications of laparoscopic operation for interstitial tubal pregnancy by clinical case analysis, and compare the efficacy and prognosis of two surgical methods. [Methods] Forty patients with interstitial tubal pregnancy admitted to our hospital from Jan. 2006 and Dec. 2007 were analyzed. Among all patients, 18 patients underwent laparoscopy and 22 patients underwent taparotomy. Two groups were compared. [Results] There was no statistical difference in operative time, the decrease of serum β-HCG in 24 hours after operation and the time of serum β-HCG turning to negative between two groups（ P〉0. 05）. But there was statistical difference in postoperative hospital day, the time of urethral catheter reservation, the time of passage of gas by anus, postoperative analgesia and hospitalization cost between two groups（ P〈0.05）. [Conclusion] Compared with laparotomy, laparoscopic operation for interstitial tubal pregnancy is safe and feasible with the advantages of less invasion and rapid recovery, so it is worthy of being extended.
