急性脑梗死患者血浆同型半胱氨酸与血小板活化的相关性研究

摘    要:目的观察急性脑梗死患者血浆同型半胱氨酸(Hcy)、血小板活化指标P选择素、血栓烷B2水平及相互关系,从而研究高同型半胱氨酸(HHcy)血症促进脑梗死发生的机制。方法选择急性脑梗死患者(脑梗死组)466例,再将脑梗死组分为HHcy患者345例和非HHcy患者121例;另选择健康体检者62例为对照组。检测Hcy、P选择素、血栓烷B_2水平,比较Hcy与P选择素、血栓烷B_2的相关性。结果与对照组比较,脑梗死组患者血浆Hcy、P选择素、血栓烷B_2水平明显升高(P<0.05);与非HHcy患者比较,HHcy患者P选择素、血栓烷B_2水平明显升高(P<0.05);脑梗死组、HHcy患者Hcy与P选择素、血栓烷B_2呈正相关(r=0.223,r=0.195,r=0.227,r=0.151,P<0.05);非HHcy患者Hcy与P选择素、血栓烷B_2无相关性。结论 HHcy是脑梗死的危险因素,它可能通过激活血小板的途径导致血栓形成。

Correlation between blood plasm homocysteine and platelet activation in the patients with acute cerebral infarction
Abstract:Objective To study the mechanism of hyperhomocysteinemia(HHcy) to facilitate acute cerebral infarction by observing the levels of blood plasm homocysteine(Hcy),P-selectin and thromboxaneB_2(TXB_2) and their relationship.Methods 466 acute cerebral infarction patients were divided into 3 groups-HHcy(n=345) and non-HHcy(n=121),and normal control(n=62), and the levels of Hey,P-selectin and TXB_2 in the three groups were compared.The correlation between Hcy,P-selectin and TXB_2 were studied in the three groups.Results The levels of Hcy, P-selectin and TXB2 in cerebral infarction patients were higher than in the control group(P< 0.05).The levels of P-selectin and TXB_2 in HHcy group were higher than in non-HHcy group (P<0.05).Hcy was correlated with P-selectin and TXB_2 in cerebral infarction patients and HHcy group(r=0.223,r = 0.195,r=0.227,r=0.151,P<0.05),but not in non-HHcy group. Conclusions HHcy was the risk factor of cerebral infarction.Possibly,HHcy activated platelet and resulted in thrombogenesis.
