摘    要:目的检测老年亚临床甲状腺功能减退(亚甲减)患者的QTc和T波峰末间期,以了解亚临床甲状腺功能异常对心室复极化的影响。方法选择老年亚甲减患者(亚甲减组)43例,另选健康者(对照组)45例。2组均详细询问病史、系统体检,采集静脉血检测促甲状腺素(TSH)、游离三碘甲状腺原氨酸(FT_3)、游离甲状腺素(FT_4);行心电图检查,人工测量QT间期和T波峰末间期(Tp-Te)。结果与对照组比较,甲减组患者FT_4、心率明显降低,差异有统计学意义(P<0.05),TSH水平明显升高,RR间期、QT、QTc、Tp-Te间期明显延长.差异有统计学意义(P<0.05)。结论亚临床甲减可引起QTc和Tp-Te间期明显延长,对心室复极化有影响。

Influence of subclinical hypothyroidism on ventricular repolarization in the aged
Abstract:Objective To examine the influence of subclinical hypothyroidism on ventricular repolarization measured by corrected QTc in the aged with standard 12-lead electrocardiogram.Methods The subclinical hypothyroidism group consisted of 43 aged patients,the controls were 45 healthy aged individuals.All patients were examined for medical history and careful physical examination. Blood samples were taken from a peripheral vein after an overnight fast in the morning. The serum concentrations of TSH,FT_4,FT_3 were measured.An electrocardiogram was taken with the ECG recorder.Before ECG,the patients and controls were subjected to identical resting period of 30 minutes.The QT and Tpeak-Tend interval were measured manually.Results The FT_1 level and heart rate of subclinical hypothyroidism group were lower than those of the control group(68.49±8.36)/min vs(72.50±9.45)/min,P<0.05].TSH level was higher,QTc,RR interval, QT and Tpeak-Tend interval were longer in subclinical hypothyroidism group than those in the control group.The differences were significant(P<0.05).Conclusion QTc and Tpeak-Tend interval were significantly prolonged in patients with subclinical hypothyroidism.Subclinical hypothyroidism can influence ventricular repolarization.
