2型糖尿病患者血清脂联素、肿瘤坏死因子α水平与颈动脉内膜中层厚度的关系

摘    要:目的探讨2型糖尿病(T2DM)患者血清脂联素(APN)、肿瘤坏死因子α(TNF-α)水平与颈动脉内膜-中层厚度(IMT)的关系。方法对120例T2DM患者的大血管病变危险因素进行采集,应用B超技术测量其IMT,同时测定空腹血清APN和TNF-α水平,观察IMT增厚组(IMT≥1mm,n=56)与不增厚组(IMT〈1mm,n=64)血清APN和TNF-α水平与IMT的关系。结果 T2DM患者IMT增厚组血清TNF-α高于IMT不增厚组(P=0.0018),血清APN低于IMT不增厚组(P=0.0014),相关分析显示,IMT与TNF-α呈正相关(r=0.45,P〈0.01),与APN呈负相关(r=-0.51,P〈0.01)。结论 T2DM患者IMT增厚与空腹血清APN水平降低、TNF-α水平升高明显相关,提示血清APN和TNF-α水平变化参与颈动脉粥样硬化的发生和发展过程,与颈动脉粥样硬化性狭窄关系密切。

Abstract:Objective To investigate the relationship between serum adiponectin, tumor necrosis factor-α and carotid artery intima-media thickness in type 2 diabetic patients. Methods 120 patients with type 2 diabetic were divided into control groups ( IMT〈1 mm,n= 64) and observation group ( IMT≥1 mm,n=56). Carotid artery IMT were measured by ultrasound and the levels of serum APN, TNF-α were measured. The relationship between serum adiponectin, tumor necrosis faetor-α and carotid artery intima-media thickness was Observed. Results Serum levels of TNF-α in observation group were significantly higher than that of the control group(P= 0. 0018). Serum levels of APN in observation group was significantly lower than that of the control group(P=0. 0014). Correlation analysis showed that TNF-α was positively associated with carotid artery IMT(r=0.45, P〈 0.01) and APN was negatively associated with carotid artery IMT(r=0.51 ,P〈0.01). Conclusion The carotid artery IMT in type 2 diabetic patients is associated with serum APN and TNF-α. TNF-α contribute to the occurrence of carotid artery atheroscterotic and APN is protective factors of carotid artery atherosclerotie.
